In 
order to qualify for group coverage with Health Plan Select you must be a small or large business with a minimum of two full time employees.
Even though the minimum requirement is two employees, more employees could be required to participate depending on the number of eligible employees the group has, based on participation requirements.
Please refer to the Underwriting and Eligibility Requirements for more detailed information on becoming a new group. Because each group has different demographics you can obtain preliminary rates based on age and gender; however, rates are not final until the group has been processed through medical underwriting.
