The Healthy Returns Care Management Program (HRCM) has introduced an enhanced Web site called My Nurse Online.
The site, www.healthy-returns.mynurseonline.com offers health information and tools for those diagnosed with CAD, CHF or diabetes. Through the simple click of a button, HRCM program participants will now be able to update their personal health data or gain access to a wide variety of health information and tools online to manage their health more effectively. My Nurse Online is available 24 hours a day, 7 days a week.
All information transmitted through My Nurse Online is kept secure and confidential.
